Home Care CNA

To ensure delivery of safe, compassionate, high quality personal care, working under the supervision of the Team Leader or Case Managing nurse in accordance with the treatment plan established for each specific patient within the Home Care System. Acts as an advocate for home care in their respective communities and adheres to the hospital Mission, Vision, Values and Motto of “The patient is First in all We Do”.

ESSENTIAL DUTIES

Clinical

  1. Will perform patient Activities of Daily Living (ADL’s), Instrumental Activities of Daily Living (IADL’s) and comfort measures as outlined by the plan of care and service policies and procedures essential to health care and safety in the home.
  2. Under the direction of the case managing nurse, the aide may assist in simple procedures and perform tasks as instructed.
  3. Knowledge and competency of the aide to perform the procedure or task must be demonstrated to the RN.
  4. May help instruct family in proper ways to accomplish these tasks as directed by the case managing nurse.
  5. Assist patient to achieve maximum self-reliance.
  6. May assist with medications that are ordinarily self-administered as directed by plan of care.

Documentation

  1. Completes assigned logs and appropriate visit notes related to patient care in a timely manner according to agency policy.
  2. Completes a weekly schedule of patients assigned and works closely with HHA coordinator to assure patients are seen according to the care plan.

Supervision

  1. Will report changes in the patient’s condition to the case managing R.N. or supervisor and will participate in case conferences of patient care with other team members.
  2. Reports to the office in a timely manner when unable to see patients due to illness or for other reasons.
  3. Assists HHA coordinator to cover patient care when planned time off is taken.
  4. Willingly participates in required supervisory visits as required by law.

Agency Promotion

  1. Completes Home Health Aide Requirements yearly, including but not inclusive to;
  2. Twelve hours in-service,
  3. Demonstration of competency by passing a yearly written exam and skills test provided by agency.
  4. Demonstrates a spirit of cooperation and enthusiasm in order to create a team atmosphere conducive to self-improvement and agency growth.
  5. Performs other duties and responsibilities as assigned by the home care director, participating in specialty teams and agency programs.

 

 

QUALIFICATIONS

Required Job Qualifications include:

  1. High school graduate.
  1. Completed the CNA training course as outlined by the State, and have passed the State Office of Education Certified Nurses Aide Certification program.
  2. One year of health care related experience.  Homecare experience preferred.
  3. Must be 21 years of age with a valid driver license, good driving record, and transportation.
  4. Have a good physical and mental health with a positive attitude and good temperament in caring for ill and difficult patients.
  5. Have the verbal ability to communicate with patience and tact when dealing with patients, physicians and staff members.
  6. Understands the aging process, and emotional problems of illness, with a genuine interest in geriatric nursing.
  7. Has an acute sense of responsibility and moral integrity. Able to identify problems and assist in being part of the solution rather than the problem.
  8. Able to perform duties to the best of her/his ability and adjust to a working environment, which will change with each individual patient. Must be non-critical of patient’s environment, which will change with each individual patient. Must be non-critical of patient’s life style and home environment.
  9. Acts as a self-starter who works independently and who will consistently find ways to improve their own job and department operations as part of the home care team.
  10. Assumes responsibility for knowing agency policy and expectations.
  11. Must attend monthly staff meetings and yearly OSHA training.
  12. Must comply with HIPPA and keep home care / hospital confidentiality.

 

PHYSICAL DEMANDS/REQUIREMENTS

While performing the duties of this position, the employee is frequently required to carry, climb, hear/listen, lift, reach and manipulate objects, tools, or equipment, push/pull, see, speak, squat/kneel, stand, and walk.  Heavy physical exertion is also required as duties involve moving materials weighing 10 – 20 pounds continually, and 25 – 50 pounds on an occasional basis, along with lifting patients, for patient care.  Manual dexterity and coordination are required over 80% of the work period.  Work involves the potential for exposure to infectious and patient’s pets.  This position requires mobility. The employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
